    In 1996, the Hospital Council Santa Clara Section began to fund and implement a community benefits project focusing on Latinos living with diabetes. The emphasis was on developing and improving access to education and primary care services. This effort gave birth to the Community Diabetes Project, a program of Community Health Partnership. Primary Care Services

Our programs place a particular emphasis on the critical role of the family and respect for the identities and the well being of various ethnically diverse populations. We serve children and families, adolescents, mentally ill adults, homeless, and older adult populations. Read our Gardner Brochure.

Gardner's health clinics provide services in English, Spanish, Vietnamese, Tagalog, French, and Russian.

Gardner accepts Medicare, Medi-Cal, Healthy Kids, Healthy Families. Sliding Scale fees are available for uninsured patients. We can assist with Medi-Cal applications and other financial assistance programs.
